Fill in DIY Set Up Documents

Steps: 
1. Press "DIY Set up documents"
2. Press "Read and Continue" after reading the conditions and terms
   
Basic Information: 
3. Enter English Company Name, For example: "SAMPLE LIMITED"
4. Enter Chinese Company Name, For example:「樣本有限公司」- if applicable
5. Choose "Package A" 或 "Package B"
6. Enter Registered Office Address,
  For example, "Room 1234, 12/F., ABC Building, 888 Company Street, Hong Kong."
7. Check the box if you use our provided address
8. Enter contact details. "Contact"﹐"Email"and "Mailing Address" MUST be filled in.
  For example,
  「Contact」: "John SMITH"
  「Phone」: "1-123-456-789"
  「Email」:"johnsmith@johnsmith.com
  「Mailing Address」:"Overseas Mansion, 333 Overseas Street, U.S.A."
   
Directors and Shareholders information: 
9. Enter Director/Shareholder chinese name「中文姓名」- if applicable
  「中文姓名」, for example: 史約翰
10.  Enter Director/Shareholder english name「Surename」
  「Surename」, for example: SMITH
11.  Enter Director/Shareholder english name「Given Name」
  「Given Name」, for example: John
12.  Enter Director/Shareholder「Residential Address」and 「Country」
  「Residential Address」, for example : Overseas Mansion, 333 Overseas Street
  「Country」, for example : U.S.A.
13.  Enter Shareholder's「No. of shares」
  「No. of shares」, for example : 5000
14.  Enter Director/Shareholder identification data.
  Choose either one from "HK ID Card No."﹐"PRC ID Card No." and "Passport No."
  If you choose "Passport No.", please also enter "Issue Country".
  For example:
  「HK ID Card No.」: A1234567(8); OR
  「PRC ID Card No.」: 666677778888; OR
  「Passport No.」: NT123456 and 「Issue Country」: U.S.A.
15. If there is a second Director/Shareholder, please fill in according to step 9~14. 
16.  Make sure you enter all necessary date correctly, then enter the "security code" 
17.  Then press "Print Form" 
   
Print and Sign
Steps:
1. Save the PDF file on your computer.
2. Open the saved PDF file.
3. Print out the PDF file. (using A4 size paper and print on ONE-SIDE only)
4. Check to see if all entered data are correct.
5. Sign on page 5, 7, 8 and 9 (only 1 Director/Shareholder)
6. Sign on page 5, 7, 8, 9 and 10 (2 Directors/Shareholds)
7. Mail us the signed orginal documents and make payment through Paypal or Bank Transfer
